Our home monitoring falls prevention service aims to reduce the risk of falls in those vulnerable. Healthcare professionals are able to monitor their patient’s risk of falls using health indicators such as strength, and completion of balance exercises.

The falls prevention pathway is simple to use for both the healthcare professional and the patient. Our service involves the patient receiving an automated phone call, where they are asked a few simple questions based around whether or not they have had any falls, how they feel about their balance and how they are getting on with their home exercise plan. The patient indicates their response using their telephone keypad. The frequency of the calls and the questions asked can be tailored for each patient. If the patient fails to respond, a health professional is alerted so contact can be made.

The data the patient provides is shared with their care team, where clinicians will be made aware of any changes in their patient’s status.
