The Health Call long term conditions service enables health professionals to treat patients with a range of long term health conditions at scale.

The long term conditions service can improve the monitoring and management of conditions such as COPD and heart failure. The service enables patients to provide their care teams with daily updates of health indicators such as blood pressure, heart rate, SP02 and weight from the convenience of their own home.

The long term condition service uses a digital device such as a blood pressure monitor which allows the patient to self-monitor. The patient can then use automated phone technology to record their readings. At a pre-agreed date and time, the patient receives a phone call. The patient uses the telephone keypad to enter their readings. The service can also ask the patient questions around their general health and well being.

Results can be sent directly to a triage team or to a patient’s care team. If results show any reason for concern, the patient can be immediately contacted to arrange changes in treatment or make adjustments to their care plan.

The way in which this data is integrated is by a digital health platform. The platform enables patient readings to flow into GP clinical systems such as EMIS Web and SystmOne. Patient readings can flow into the system with just one click up a button, meaning health professionals always have access to their patients data.

If delivered properly, telehealth can substantially reduce mortality, reduce the need for admissions to hospital, lower the number of bed days spent in hospital and reduce the time spent in A&E - Whole system demonstrator programme: Headline findings: December 2011
